New Patients
If you are new to the practice, and you have already called to schedule an appointment, we ask that you please click on the link below to complete your Health History form... from the privacy and convenience of your own home. It is very easy (and secure) and should take you about 5 to 10 minutes to complete. If you have any questions, please call our office at 785-594-9834, and we will be happy to help.
(NOTE: Please do not worry about filling out your Insurance information on this form; we will get it from you by phone or at the office. If you are unable to complete the form online, it's okay! Please just arrive at least 15 minutes early for your scheduled appointment to complete the form at our office.)
Insurance & Financial Information
Our practice is happy to submit claims to any insurance company. We are contracted providers with Delta Dental and Blue Cross/Blue Shield of Kansas, and we are happy to submit claims and accept assignment of benefit from most other companies, including Aetna, Cigna, Coventry, Humana, Met Life, United Concordia, and many others.
We will do our best to help you understand the details of your particular insurance, but it is important that you have an understanding of how your insurance works, and that you ask us questions about this confusing subject if you do not have a thorough understanding.
We will do our best to help you afford the dental care you need. We gladly accept Cash, Check, Visa, MasterCard, and Discover for payment. We can arrange short-term payment plans, as well as long-term, interest-free financing through CareCredit, upon approval. Learn more at www.CareCredit.com.
